Complete the following information if you would like to make a request to use space at First Presbyterian Church. Click here to view the facility fee schedule. You may submit the basic information of your request through the online form below, but the church will need signed copies of the Hold Harmless Agreement, the Agreement of Responsibilities, and the Protecting our Children if your group supervises minors, prior to approval. These forms may be printed and mailed to First Presbyterian Church (116 South Loudoun Street; Winchester, VA 22601), scanned, and emailed to Andrea Barron, firstname.lastname@example.org, or will be accepted in person at the church Monday-Friday 8:30-4:30.

PROTECTING OUR CHILDREN POLICY If your facility use will involve supervising minors, the leader of your group will need to read, sign, and return the final page of the FPC Protecting Our Children Policy. By consenting, you are responsible for informing group leaders and enforcing the “2-Leader Rule”, “Open Classrooms”, and have your own procedures in place for screening volunteers and leaders who will be responsible for working with young people while you are using First Presbyterian Chuch facilities.
COMPLETION OF THIS FORM IS NOT CONFIRMATION OF APPROVAL. YOU WILL RECEIVE NOTIFICATION FROM THE CHURCH IF YOUR APPLICATION HAS BEEN APPROVED OR DENIED. IF YOUR REQUEST IS APPROVED, YOU MAY BE SUBJECTED TO FEES AS DETERMINED BY FACILITIES COUNCIL. NON-PAYMENT OF THE SET FEES PRIOR TO YOUR EVENT WILL RESULT IN AUTOMATIC CANCELLATION OF YOUR EVENT AT THE CHURCH.
IF YOU DO NOT RECEIVE NOTIFICATION OF THE OUTCOME OF YOUR APPLICATION, PLEASE CONTACT THE CHURCH OFFICE FOR INFORMATION.
WE RECOMMEND TO NOT PUBLICIZE YOUR EVENT UNTIL YOU HAVE RECEIVED CONFIRMATION FROM THE CHURCH: YOUR EVENT’S DATE, TIME, AND LOCATION, ON OUR CALENDAR; IF ANY, PAYMENT REQUIRED FOR THE FEES; AND ANY OTHER INSTRUCTIONS, SUCH AS ACCESS TO YOUR REQUESTED SPACE.
Facility Use RequestThis form must be completed in full and submitted for review and action by the Facilities Council of First Presbyterian Church, which meets monthly on the 2nd Tuesday of each month.
